The Waldwick, NJ office is open, and Signature Care (in-office + telehealth) and standalone OMT visits are also available for patients who prefer office-based care. Telehealth is available statewide to any patient located in New Jersey at the time of their visit. Service availability is determined by location and may not be available in every community.

Through her Concierge Care, you get in-person care at home — sick visits, wellness, annual physicals — plus all telehealth benefits, with the same physician every time and no insurance billing.
Which North Jersey towns do you serve?
Home visits are available to Concierge Care members in affluent Bergen and Sussex County communities, roughly within an hour of Newfoundland, NJ. These include Sparta, Franklin Lakes, Wyckoff, Mahwah, Saddle River, Upper Saddle River, Glen Rock, Allendale, Waldwick, Ridgewood, Ramsey, and nearby communities. The Waldwick, NJ office is open for Signature Care and OMT visits. Telehealth is also available statewide to any patient located in New Jersey at the time of their visit.
Do you offer OMT?
Yes. As a DO with osteopathic training, Dr. Nelly delivers osteopathic manipulative treatment (OMT) at the Waldwick, NJ office. OMT can help with back and neck pain, joint stiffness, headaches, muscle tension, and injury recovery. Pricing is $250 for the initial visit and $160 for follow-ups, and no membership is required. Is home-visit care concierge medicine?
It shares many features of concierge medicine — longer visits, direct access, and home visits — but it's structured as concierge-style direct primary care, which is designed to be more transparent and accessible. Concierge Care is $450/month for an individual or $1400/month for a household (2 adults, can include up to 2 dependents). It adds in-person home visits and after-hours access on top of in-office and telehealth care, and requires a complimentary meet & greet to enroll. There's no insurance billing, no copays, and you can cancel anytime.
